Your first home visit is a full preventive assessment. With your annual panel results, Dr. Karla Cornejo interprets your situation and gives you a bilingual summary with your preventive priorities for the next 12 months.
That action plan guides the rest of your year: what to watch, which studies apply and when to check in again.

Lab tests
Your annual panel brings together the tests that establish your starting point. Dr. Karla Cornejo interprets them and turns them into your personalized action plan.

Once per benefit year, at enrollment and at each renewal. These 19 tests make up the panel for every membership.

2. Targeted check-ups
Essential does not include additional targeted check-ups during the year. An extra check-up may only be considered on medical indication in specific circumstances; it is not guaranteed and, if it falls outside coverage, it is quoted and approved by you before it is done.
3. Screenings
Essential includes guidance: your doctor reviews any pending vaccines and screenings and tells you which ones apply to you. Screening tests are not included in Essential and are quoted before they are done.
Dr. Karla Cornejo determines laboratory tests according to each patient's needs. Tests require a medical order issued by Dr. Karla Cornejo in the patient's name.

- Studies, tests or services outside plan coverage: quoted and approved by you first
- Specialist visits, imaging and outside procedures
- Ambulance, emergency care and 24/7 availability
Good to know
- $95 a month for the main member and $75 a month for each additional member at the same level.
- Each person can use their benefits for twelve months from activation, while the membership is active. Unused benefits do not roll over.
- An additional member who joins later starts their own twelve months, and their billing is combined with the main member's.
- The initial assessment uses one of your home visits.
- Scheduled care from 8:00 a.m. to 6:00 p.m. Visits outside those hours, on weekends or outside our standard area carry a fee shared with you before booking.
- You can reschedule at no charge with 24 hours' notice.
- You can move up a level whenever you need to; moving down takes effect at renewal.
- Your membership starts once the first payment and availability are confirmed. Before that, you receive the payment and cancellation rules in writing.
