Initial assessment and action plan

Your first home visit is a full preventive assessment. With your annual panel results, Dr. Karla Cornejo interprets your situation and gives you a bilingual summary with your preventive priorities for the next 12 months.

That action plan guides the rest of your year: what to watch, which studies apply and when to check in again.

Lab tests

Beyond the annual panel, Preferred includes a targeted mid-year check-up. Dr. Karla Cornejo selects its tests for your case and builds them into your personalized action plan.

1. Annual panel

Once per benefit year, at enrollment and at each renewal. These 19 tests make up the panel for every membership.

Blood and metabolism

2. Targeted mid-year check-up

Between months 4 and 6 you have a targeted check-up. Dr. Karla Cornejo decides its tests based on your history, medications and initial results, and compares your progress with your starting assessment.

3. Preventive screening: one strategy

Preferred includes one applicable screening strategy, when appropriate for your case. It is a single strategy from these options; for cervical screening, one option is chosen, not all three.

  • FIT (fecal immunochemical test) for colorectal screening
  • PSA (prostate-specific antigen), after shared decision-making
  • Pap test, high-risk HPV test or co-test: one option, not all three
  • Hepatitis C, if no previous screening has been documented

Dr. Karla Cornejo determines laboratory tests according to each patient's needs. Tests require a medical order issued by Dr. Karla Cornejo in the patient's name.

We guide and support you

  • Coordination of appointments and follow-up with specialists or outside services

This support does not include paying for the outside study or visit.

Not part of the membership

  • Studies, tests or services outside plan coverage: quoted and approved by you first
  • Specialist visits, imaging and outside procedures
  • Ambulance, emergency care and 24/7 availability

Good to know

  • $145 a month for the main member and $120 a month for each additional member at the same level.
  • Each person can use their benefits for twelve months from activation, while the membership is active. Unused benefits do not roll over.
  • An additional member who joins later starts their own twelve months, and their billing is combined with the main member's.
  • The initial assessment uses one of your home visits.
  • Scheduled care from 8:00 a.m. to 6:00 p.m. Visits outside those hours, on weekends or outside our standard area carry a fee shared with you before booking.
  • You can reschedule at no charge with 24 hours' notice.
  • You can move up a level whenever you need to; moving down takes effect at renewal.
  • Reports for family members require your authorization.
  • Your membership starts once the first payment and availability are confirmed. Before that, you receive the payment and cancellation rules in writing.
