Initial assessment and action plan

Your first home visit is a full preventive assessment. With your annual panel results, Dr. Karla Cornejo interprets your situation and gives you a bilingual summary with your preventive priorities for the next 12 months.

That action plan guides the rest of your year: what to watch, which studies apply and when to check in again.

2. Targeted check-ups

Plus includes up to two targeted check-ups a year: the first between months 4 and 6 and the second between months 8 and 10. Dr. Karla Cornejo decides their tests based on your assessment and records them in your prevention plan. Not every possible test is performed for every person.

3. Screenings and options

In Plus, screening is personalized by age, sex, history and expected benefit. The options are alternatives: for cervical screening, one is chosen, not all three. Studies coordinated with outside providers, such as mammography or colonoscopy, are not included.

  • FIT (fecal immunochemical test) for colorectal screening
  • PSA (prostate-specific antigen), after shared decision-making
  • Pap test, high-risk HPV test or co-test: one option, not all three
  • Hepatitis C, if no previous screening has been documented

Dr. Karla Cornejo determines laboratory tests according to each patient's needs. Tests require a medical order issued by Dr. Karla Cornejo in the patient's name.
